We offer residents several types of living arrangements, including independent living, continuing care retirement communities, assisted living, memory care and skilled nursing care. We help older adults maintain their independence, health and well-being in a safe and comfortable environment. Residents can trade in the uncertainties of living alone and the responsibilities of maintaining a home for theLiving Room setting affordable services that simplify life.
 
Independent Living
Independent living is for adults who want to enjoy the benefits of living in an active and safe environment without the hassles of home ownership.  Horizon Bay’s independent living communities offer services and amenities - scheduled transportation, dining services, resident programs, housekeeping and transportation – that are geared toward fostering independence and creating more socializing opportunities.
 
Continuing Care Retirement Community
A continuing care retirement community (CCRC) offers several levels of assistance, including independent living, assisted living and skilled nursing care. It is different from other retirement communities because it usually provides a written agreement or Life Care contract between the resident and the community that offers a continuum of housing, services and health care, commonly all on one campus or site.  Click on one of the CCRCs below for more information.

Assisted Living
Assisted living is a special combination of apartment living, personalized supportive services and health care designed to assist residents with the activities of daily living. We assist with activities such as dressing, eating, bathing, shopping and laundry. Assisted living offers more health care services than independent living communities.
 
Memory Care
Our Memory Care communities cater to people who have Alzheimer’s disease or related disorders. Residents live in a separate, secure area with many of the same amenities as assisted living residents, such as apartments, dining, resident programs and housekeeping. 
 
Skilled Nursing Care
Select communities offer skilled nursing care, which is for residents who require 24-hour care and attention that goes beyond basic assisted living services. Our skilled nursing communities are located in Florida, Colorado, Rhode Island and Texas, and are licensed by each state’s Department of Health Services. 

What are Continuing Care Retirement Communities (CCRCs)?

Continuing Care Retirement Communities, also known as life care facilities, are retirement communities where a number of aging care needs may all be met in a single residence.  The types of care you will find include independent living, assisted living and skilled nursing. 

Typically, new residents move into a continuing care retirement community (CCRC) while still living independently with few health risks or healthcare needs, and will reside there until the end of life. As residents progress in age, and medical needs change, the level of nursing care and service increase in such a way, the residents’ needs are monitored and addressed. If greater illness or injury warrants hospitalization, the resident may return to his or her apartment after recovery, and will receive appropriate treatment and care.

Horizon Bay’s CCRCs offer a life care contract, which combines active community living with the confidence of knowing that assisted living care, skilled nursing care and home health are available should the need arise.
